Home and Community Care Support Services (HCCSS) is a non-profit crown agency legislated and funded by the Ontario Ministry of Health that supports higher needs patients and provides in-home care, long-term care placement coordination, and partners with Community Support Services to connect people to community support services across Ontario.
The cost of the services provided by HCCSS are 100% covered through government funding.
HCCS is often the first point of contact to connect people to a broad range of community and social supports.
HCCSS Care Coordinators, located in the community, hospitals, or primary care practices work closely with patients to determine care needs and monitor each patient's care plan. If you quality for HCCSS services, you will be assigned your own personal care coordinator.
Support provided by HCCSS is unlimited, based on need and availability.
